Rehabilitación oral en paciente pediátrico con patologías sistémicas: Microcefalia, Autismo y Trastorno Global del Desarrollo: Reporte de Caso.

Abstract

Los pacientes pediátricos con enfermedades neurológicas siempre serán un reto dentro de la atención odontológica, tanto para el especialista como para sus padres o personas a cargo, debido a que su manejo es completamente diferente a comparación de los niños sin algún tipo de afectación neurológica. Dentro del primer semestre del año 2021 se empezó el tratamiento del paciente de sexo masculino de 4 años, quien presenta diversas patologías sistémicas como: trastorno global del desarrollo, microcefalia y autismo. Se realizó la valoración del paciente el cual presentaba lesiones cariosas con afección pulpar, caries incipientes en dientes anterosuperiores, caries sin afección pulpar y macroglosia. Con el diagnóstico obtenido, se procedió a la rehabilitación del paciente con la colocación de coronas metálicas, restauraciones de ionómero de vidrio y carillas estéticas a base de resina. La ejecución del plan de tratamiento tuvo varias fases, al principio fue un proceso de establecer un nexo con el paciente con el fin de obtener una relación de confianza. Se modificó la atención con la adaptación de los tratamientos y las citas en cuanto a tiempo y postergación, debido a las características propias del paciente y su estado de salud que en ocasiones no era el óptimo. La rehabilitación oral del paciente fue exitosa, a pesar de las variaciones que se presentaron a lo largo del tratamiento, se pudo devolver la funcionalidad masticatoria de los dientes posteriores temporales para una mejor alimentación al igual que se otorgó estética a los dientes anterosuperiores temporales. Pediatric patients with neurological diseases will always be a challenge in dental care, both for the specialist and their parents or caregivers, because their management is completely different compared to children without any type of neurological involvement. Within the first semester of 2021, the treatment of the 4-year-old male patient, who presents various systemic pathologies such as: global development disorder, microcephaly and autism. The evaluation of the patient was carried out, which presented the following: carious lesions with pulp involvement, incipient caries in upper anterior teeth, caries without pulp involvement and macroglossia. With the diagnosis obtained, the patient was rehabilitated with the placement of metal crowns, glass ionomer restorations and aesthetic resin-based veneers. The execution of the treatment plan had several phases, at first it was a process of establishing a link with the patient in order to obtain a relationship of trust. Care was modified with the adaptation of treatments and appointments in terms of time and the postponement of these, due to the particular characteristics of the patient and their health status, which sometimes was not optimal for their care. The patient's oral rehabilitation was successful, despite the variations that occurred throughout the treatment, it was possible to restore the masticatory functionality of the temporary posterior teeth for better feeding, as well as aesthetics to the temporary upper anterior teeth.
