Tratamiento estético de gingivectomía en paciente ortodóntico que desarrolla hiperplasia gingival asociada a acumulación de placa bacteriana

Abstract

El agrandamiento gingival se define como una alteración del tejido conectivo, en la cual se observa un crecimiento excesivo de la encía por un aumento del número de sus componentes celulares. En el presente caso clínico, paciente ortodóntico de género femenino de 39 años acude a consulta refiriendo “molestia en las encías” e inconformidad estética y funcional en la zona anterior debido a agrandamiento gingival. El objetivo del presente trabajo fue restablecer la estética y funcionalidad del periodonto de protección al paciente ortodóntico con hiperplasia gingival asociada a la acumulación de placa bacteriana. Para ello, se estudió la etiología de la patología desarrollada, diagnosticando la enfermedad como un agrandamiento gingival inflamatorio crónico inducido por el acúmulo de biofilm y agravado por la presencia de aparatología ortodóntica fija. A continuación se estableció un plan de tratamiento adecuado, combinando la fase higiénica, correctiva y de mantenimiento. De esta manera, se restableció la estética y funcionalidad del periodonto de protección en el paciente ortodóntico, llevando a cabo una gingivectomía estética mediante la técnica quirúrgica convencional. Gingival enlargement is defined as an alteration of the connective tissue, in which an excessive growth of the gum is observed due to an increase in the number of its cellular components. In the present clinical case, a 39-year-old female orthodontic patient came to the consultation referring to “discomfort in the gums” and aesthetic and functional discomfort in the anterior area due to gingival enlargement. The objective of the present work was to restore the aesthetics and functionality of the protective periodontium to the orthodontic patient with gingival hyperplasia associated with the accumulation of bacterial plaque. For this, the etiology of the developed pathology was studied, diagnosing the disease as a chronic gingival enlargement induced by the accumulation of biofilm and aggravated by the presence of fixed orthodontic appliances. A suitable treatment plan was then established, combining the hygienic, corrective and maintenance phase. In this way, an aesthetic gingivectomy was carried out using the conventional surgical technique, thus restoring the gingiva to its natural state and restoring aesthetics and functionality to the protective periodontium.
