Devolución de la estabilidad oclusal y estetica en paciente Edentulo total con falsa clase III de Angle con protesis totales

Abstract

La pérdida de dientes es un problema que aqueja a muchas personas hoy en día, las personas no se sienten a gusto por su apariencia y acuden al especialista, es por eso que la confección de prótesis totales es una alternativa para reponer los dientes perdidos y mejorar la fisonomía del paciente. En este trabajo se rehabilitó una paciente de 54 años de edad, sexo femenino edéntula total, la cual al llegar a la consulta después de la valoración clínica se observa una clase III de Angle con respecto a las prótesis totales que la paciente estaba usando, y también se sospecha de una clase III esqueletal por lo que se decide realizar radiografía lateral de cráneo y Cefalometría. Las prótesis totales la paciente tenía desde aproximadamente 10 años, estas prótesis comenzaron siendo parciales y conforme fue perdiendo el resto de sus dientes se fueron adaptando mas dientes sobre esta misma estructura poco a poco. Es así que estas prótesis por haber sido adaptadas paulatinamente no presentaban oclusión adecuada y estéticamente la paciente refirió inconformidad. Se opto rehabilitar a la paciente con nuevas prótesis totales que se adapten al reborde residual y que cumplan con la estética y la oclusión adecuada. El tratamiento fue satisfactorio, por medio del trazo cefalométrico se verificó que la paciente tiene una clase I esqueletal por lo que se decidió cambiar la aparente clase III a clase I, y por consiguiente mejoramos la estética y corregimos la oclusión. La paciente queda conforme con los resultados obtenidos y pudo adaptarse sin problema a sus nuevas prótesis.

Tooth loss is a problem that afflicts many people today, people do not feel comfortable about their appearance and therefore they attend to a specialist. This is the reason why the making of dentures is an alternative to replace missing teeth and improve the appearance of the patient. In this work we rehabilitated a 54 year-old women total edentulous, which upon reaching consultation had Angle Class III based on total prosthesis, and also suspected of a skeletal class III so it was decided to perform lateral skull radiography and cephalometric. The patient had full dentures for about 10 years, this started out as partial dentures and as she lost the rest of her teeth, gradually more teeth were adapted on the structure. Due to this procedure, the prostheses were not properly prepared and did not have proper occlusion, which led to aesthetical discomfort and dissatisfaction for the patient. We opted to rehabilitate the patient with new dentures that fit the residual ridge and meet the aesthetic and proper occlusion. The treatment was effective; we change the apparent class III to class I, based on the results of the cephalometric and consequently improve the aesthetic and correct occlusion. The patient is satisfied with the results and was able to adapt smoothly to the new prosthesis.
