Endodontics, a branch of dentistry, which consists of a treatment of the dental pulp, which is essential to make a correct diagnosis by collecting all the signs and symptoms, the tooth to be treated in this study it is the first superior premolar, but as other teeth can present anatomical variations, being very probable that there are failures during this therapy when not everything is known correctly in the tooth to treat, becoming very important to have adequate knowledge about the anatomy dental, its morphology, anatomical variations, clinical principles, asepsis conditions, access procedures, instrumentation, irrigation, obturation, etc., using an experimental, clinical and observational method, obtaining among the results possibilities that all teeth present variations with respect to their anatomy, morphology in their roots s, conduits for which it is necessary to have a broad knowledge and learn to diagnose through the complementary examinations and all the data exposed and reduce or avoid the possibility of failure. Therefore, in this clinical case, it becomes a challenge for the clinician in his daily practice that he can present, further determining the effectiveness of the rotating system in the root canal instrumentation, its subsequent filling and rehabilitation.
