Abstract: | La oclusión es un pilar fundamental de la odontología. Para tener una terapia
oclusal exitosa es imprescindible que las articulaciones temporomandibulares se
encuentren cómodas y estables para que sean capaces de funcionar y aceptar la carga
de las fuerzas sin ningún malestar. Los dientes anteriores deben estar en armonía con
la cobertura de la función y los dientes posteriores sin interferencias, es decir no
deben ser interrumpidos por la ATM o por la guía anterior. Una patología que afecta
directamente a la oclusión es el bruxismo, al ser una parafunción se entiende que no
tiene un propósito funcional, y como producto se produce una lesión no cariosa
conocida como atrición. La atrición tiene como causa la fricción entre los dientes
antagonistas produciendo desgaste dentario. Cuando este desgaste sobrepasa las
10μm al año es considerado patológico. Por este motivo es importante considerar a la
oclusión habitual, la guía anterior y la dimensión vertical como factores importantes
al momento de una modificación. En estos casos, existen protocolos preestablecidos
para su rehabilitación, y si los pacientes no tienen las posibilidades de realizarse el
tratamiento ideal existen opciones alternas para mejorar su condición y poder
efectuarlo en un futuro. Este es el caso tratado a continuación en el cual mediante un
exhaustivo diagnóstico de la oclusión se logró encontrar el mejor tratamiento para la
atracción causada por bruxismo. Occlusion is a main pillar in odontology. To have a successful oclusal therapy it’s essential that the temporal mandibular joints are in a comfortable and stable position to be able to work appropriately and support different levels of pressure without it feeling unpleasant. The anterior teeth must be in harmony with the entire function and the posterior ones without any obstacles, meaning they should be not interrupted by the ATM or the anterior guide. Bruxism is one of the pathologies that affect occlusion directly. Given the fact that it’s a parafunction, it’s clear it doesn’t have a functional purpose, and it’s consequence would result in a non-carious lesion known as attrition. What causes attrition is the friction between the antagonists teeth during the chewing function over a long period of time, originating a dental wear out. When the attrition overpasses 10μm per year it’s consider pathological. The loss of molars and attraction are contributing factors to lose the mutually protected occlusion, which creates the necessity to modify intermaxilar relations. Given these reasons, it’s important to consider regular occlusion, the anterior guide and vertical dimension as important factors when the time of a modification comes. In these situations, there are pre-established protocols for their rehabilitation, and if the patients didn’t have the means to get the ideal treatment done, there are alternate options they could choose from, and get the definite treatment later on. |
