Diagnóstico de bruxismo y disfunción temporomandibular mediante el cuestionario DC/DTM y tratamiento multidisciplinario

Abstract

Los trastornos temporomandibulares son un grupo de disfunciones relacionadas a los músculos masticatorios, articulación temporomandibular y estructuras anatómicas aledañas. Tienen etiología multifactorial, sin embargo, dentro de los factores de riesgo que pueden desencadenarlos se encuentra el bruxismo. Este consiste en la actividad repetitiva de los músculos de la masticación cuya característica es apretar o rechinar los dientes y/o apretar o empujar la mandíbula. Ambos, representan una alta prevalencia en la población a nivel mundial. En este caso clínico, se presentó, una paciente de sexo femenino de 44 años que sufría de bruxismo y trastornos temporomandibulares (TTM), por presentar dolor de mandíbula, dolor de oído y ruidos articulares. Se utilizó un cuestionario adaptado del DC/DTM, el cual permite identificar patologías articulares, musculares o inflamatorias. Además, se tomó en cuenta el modelo biopsicosocial, por lo que se realizaron interconsultas con otros profesionales de la salud para proporcionar un tratamiento integral a la paciente. Se realizó interconsulta con otorrinolaringólogo, cirujano maxilofacial, psicólogo y fisioterapeuta. El psicólogo ayudó a evaluar la hipervigilancia, catastrofización, calidad de sueño y factores de estrés de la paciente. Además, el fisioterapeuta realizó un masaje de relajación de los músculos del cuello, cabeza y cara. El tratamiento preconizó terapias conservadoras, como orientaciones de soporte terapéuticas, placa oclusal acrílica rígida, infiltraciones y autocuidados. Con el uso de todos los medios diagnósticos mencionados, se pudo elegir un tratamiento acertado que, utilizando terapias enfocadas en la necesidad de la paciente, aliviar dolor y un tratamiento odontológico integral. Temporomandibular disorders (TMD) are a group of dysfunctions related to masticatory muscles, temporomandibular joints (TMJ), and surrounding anatomic structures. They have a multifactorial etiology, however, among the risk factors that arose them is bruxism. This consists in the repetitive activity of the masticatory muscles whose characteristic is clenching or grinding the teeth and/or clenching or pushing the jaw. Both represent a high prevalence in the worldwide population. In this clinical case, a 44-year-old female attended to the dental office. The patient suffered from bruxism and temporomandibular disorders (TMD), due to jaw pain, ear pain and joint noises. An adapted questionnaire from the DC/TMD was used, which allows the identification of joint, muscular or inflammatory pathologies. In addition, the biopsychosocial model was taken into account, so consultations with other health professionals were carried out to provide comprehensive treatment to the patient. Interconsultation with otorhinolaryngologist, maxillofacial surgeon, psychologist and physiotherapist was carried out. The psychologist helped to evaluate the patient's hypervigilance, catastrophizing, sleep quality, and stress factors. In addition, the physiotherapist performed a relaxation massage of the muscles of the neck, head and face. The treatment recommended conservative therapies, such as therapeutic support orientations, rigid acrylic occlusal plate and infiltrations and self-care. With the use of all the diagnostic methods that were mentioned, it was able to choose an adequate treatment that, using therapies focused on the patient’s need, was able to achieve pain relief and also provided a comprehensive dental treatment.
